Zusammenfassung
Anorektale Malformationen (ARM) umfassen ein breites Spektrum angeborener anorektaler
Fehlbildungen mit unterschiedlicher Komplexität und Prognose. Die Komplexität der
ARM und das funktionelle Ergebnis werden durch häufig assoziierte Fehlbildungen des
Urogenitaltrakts, der lumbosakralen Wirbelsäule sowie des Spinalkanals bestimmt.
Obwohl teratogene und auch genetische Einflüsse diskutiert werden, bleibt die Ätiologie
dieser kongenitalen Malformationen unklar. Aufgrund der Komplexität des Krankheitsbildes
sollten Kinder mit einer ARM in einem spezialisierten Zentrum für Kinderchirurgie
behandelt werden. Für die Verhinderung postoperativer Probleme und zum Erreichen des
bestmöglichen funktionellen Langzeitergebnisses sind eine hohe Expertise des behandelnden
Teams und die genaue Kenntnis der individuellen Besonderheiten der einzelnen ARM obligat.
Durch eine gute operative Versorgung, interdisziplinäre Betreuung und engmaschige
Nachbehandlung können eine hohe Rate an Stuhl- und Urinkontinenz sowie eine möglichst
normale psychosoziale Entwicklung des betroffenen Kindes erreicht werden.
Summary
Anorectal malformations (ARM) comprise a broad spectrum of congenital anorectal anomalies
with variable complexity and prognosis. The complexity of the ARM and the functional
outcome are dependent on associated urogenital, sacral and spinal cord malformations.
Although teratogenic and genetic influences are discussed, the etiology of ARM remains
unclear. Due to the complexity of ARM and associated malformations, children with
ARM should be treated in a tertiary center for pediatric surgery. High expertise and
exact knowledge of the individual features and pitfalls of every single ARM are essential
to avoid long-term sequelae and to achieve the best possible long-term outcome. Excellent
surgical and interdisciplinary care as well as a close follow-up program are the keys
to a high rate of fecal/urinary continence and a normal psychosocial development of
children with ARM.
